How information is collected
Information will usually be collected directly from you through referral and intake forms, counselling sessions, email, telephone, telehealth, the Brave Collective Studio website and other communication with the practice.
With your consent, information may also be received from another person or organisation involved in your care or support, such as a partner, parent or guardian, support coordinator, healthcare professional or referring service.
Storage and security
Brave Collective Studio takes reasonable steps to protect personal and health information from loss, misuse, interference and unauthorised access, modification or disclosure.
Client and clinical information may be stored electronically using secure practice-management and other service providers used by Brave Collective Studio.
Paper records, where used, are stored securely.
While reasonable measures are taken to protect information, no electronic transmission or storage system can be guaranteed to be completely secure.
Splose and other service providers
Brave Collective Studio uses Splose as a practice-management system to assist with referrals, client records, forms, appointments, documentation and practice administration.
Brave Collective Studio may also use other reputable third-party service providers where reasonably necessary to operate the practice, including website hosting, email, telehealth, payment processing, accounting and other administrative services.
These providers may process or store personal information as part of providing their services to Brave Collective Studio.
Where information may be stored or processed outside Australia, Brave Collective Studio will take reasonable steps to manage personal information in accordance with applicable Australian privacy requirements.
Disclosure of information
Your personal and health information will not ordinarily be shared with another person or organisation without your consent.
With your consent, information may be shared with people involved in your care or support, such as healthcare professionals, support coordinators, plan managers or other relevant services.
There may be circumstances where information can or must be disclosed without your consent, including where disclosure is required or authorised by law or where otherwise permitted under Australian privacy law.
Where possible and appropriate, this will be discussed with you.
NDIS participants
For clients accessing services through the NDIS, relevant information may be shared with your plan manager, support coordinator or other authorised person where this is necessary for service administration, payment or coordination and you have provided the appropriate authority or consent.
Brave Collective Studio will only share information that is reasonably necessary for the relevant purpose.
Accessing or correcting your information
You may request access to personal information Brave Collective Studio holds about you or ask for information to be corrected if you believe it is inaccurate, incomplete or out of date.
Some legal or professional limitations may apply to access in particular circumstances.
